Emergency Pre-registration

Dear Pet Owners, please read all of the instructions and then fill out the online form at the bottom of this page.

If your pet needs emergency care, we want you to know that Veterinary Emergency Care cares. Our goal is to make sure that we are able to get your pet diagnosed & treated quickly and with the least amount of stress for you. Having to complete the necessary paperwork during a stressful time is often difficult. To help we have developed an emergency pre-registration program called veCares; Veterinary Emergency Care Acute Response Enrollment System. This program will save critical time by eliminating the need to fill out registration paperwork in the event of an emergency. Working together, with you & your family veterinarian, we want to provide the best care possible in an emergency situation.

It is simple to pre-register your pet
1. Complete the below veCares pre-registration form for all pets in your home.
2. Explain to your family veterinarian that you have pre-registered your pet with Veterinary Emergency Care. They will be able to help you keep up to date records for your pet.
3. Keep up to date records on your pet in a safe place at home and bring them with you to Veterinary Emergency Care in the event of an emergency.

Communicate with your family veterinarian:
When you register your pet with VECARES please alert your family veterinarian. Ask them to provide you with a current copy of your pet’s medical record to include:

List of current medications Laboratory tests (baseline + past 12 months)
Chronic illnesses Drug allergies
Past 12 months of treatments
Keep your pet’s medical records in a safe place at home! Bring them with in the event of an emergency.
